Custom Layout: $29.99
Includes your photo, logo, address and description on opening homepage and your company logo on the outside of CD. (One time set up charge)

Additional CDs: $5.00 each

Have your CDs either delivered to your office or mailed to your client: No additional charge

Rush Charge: $45.00+ (call or email for quote)

Additional Services: Scanning, archiving and shredding of your old file boxes, shredding only, document management solutions. May be able to collect fees through escrow.

By Submitting this form I understand and agree to all of the following: I understand I am responsible for the accuracy of this information. I understand I must complete this form correctly (spelling etc) and that this information will be used to create my CD(s). • I understand if my CD(s) need(s) to be re-burned as a result of this information being incorrect, incomplete or illegible, an additional conversion fee will apply. • I understand Scandoc Imaging, Inc. will automatically shred my paper file 30 days after delivery of my CD(s) unless I notify Scandoc Imaging, Inc. within that 30 day period if I am not satisfied with the CD-ROM(s) or scanned paperwork. • I understand Scandoc Imaging, Inc., Inc does not check any file for accuracy or completeness; it is assumed that any item provided in a file belongs in that file. • I understand it is my responsibility to make sure my files are not co-mingled. • I understand that Scandoc Imaging, Inc., Inc will reorganize my files according to their indexing system. • I understand that Scandoc Imaging, Inc., Inc. will not combine my files with the broker’s and/or agents files or any other files.• I understand if I want my paper file returned instead of it being shredded for at no cost, I must indicate it on the Scandoc Imaging, Inc.’s order form with each file I submit.
